Service Care Solutions are recruiting for a Band 5 Primary Care Staff Nurse to work within Primary Care in Feltham, West London.

The role is working within the crisis and resolution team, so crisis home treatment experience will be required.

You will be responsible for providing an accessible individual patient assessment, treatment and referral service, whilst also monitoring the ongoing health status of patients and address identified issues. You will work with women to influence the impact on their health arising from current lifestyle choices. You will implement effextive discharge plans for transfer or release, while co-ordinating clinical practice and integrated care/treatments.

You must be a Registered Nurse with previous experience of working at a Band 5 level within this field.

The rate for this role is up to £24 per hour on a limited company (umbrella) basis, dependant on experience.

We also offer a £150 sign up bonus for any new nurses that register with our agency. This will be paid in your first pay packet.

We also have £250 referral fee bonus for any nurse you refer who we place in to work on a 3 month contract.
