Care Co-ordinator, Band 6 CPN.

Vacancy for a community care co-ordinator to work in Barnet.

Opportunity for an experienced Community Mental Health Nurse to provide effective care, assessment and specialist advice for older aged adults with mental health illness (for example dementia, anxiety and bipolar), in order to support their wellbeing and recovery.

The Team provides community-based treatment and support for older aged adults with moderate to severe mental health difficulties who live in the borough of Haringey.

The ideal applicant will have experience of carrying overall responsibility for the management of a caseload, acting as a care co-ordinator, as defined by the Care Programme Approach (CPA). Designing individual programmes of care collaboratively with patients to maximize self-management and social inclusion and reflect their own goals and aspirations. Reviewing and adjusting plans according to changing service user needs.

The successful candidate will join a small team in which there are continuous learning and development opportunities and a record of effectively supporting people with complex and long term difficulties to find a way to live their life with more satisfaction, freedom and sense of achievement.
