Location: Medway Council (Hybrid working considered)
Rate: £450 per day (via umbrella company)
Contract: Up to 6 months
Hours: Full-time for 2 months, then 2 days/week in the third month

Start Date: ASAP

About the Role: Medway Council is seeking a skilled and strategic Brokerage Team Lead to lead the design, implementation, and introduction of a new Bed Brokerage Team. This is a pivotal role focused on sourcing care for patients funded by both health and social care—ensuring an integrated, collaborative approach across systems.

You will play a critical leadership role in shaping a joint health and social care brokerage function, creating sustainable models for joint funding, governance, and performance monitoring that support both Medway Council and our NHS partners.

Key Responsibilities:
  • Design and implement a new Bed Brokerage Team that sources appropriate care placements for individuals funded jointly by health and social care.

  • Lead the creation of a fully integrated health and social care brokerage function, ensuring alignment with strategic priorities.

  • Develop sustainable joint funding mechanisms, governance frameworks, and performance reporting structures for both local authority and NHS partners.

  • Manage and motivate the existing Brokerage Team, embedding new ways of working and driving performance improvement.

  • Collaborate closely with operational managers, commissioning leads, finance teams, and providers to ensure care placements are timely, cost-effective, and person-centred.

  • Ensure that all brokerage activities comply with statutory requirements and reflect best practice in integrated care delivery.

About You: We’re looking for an experienced, solutions-focused leader who can bring together health and social care systems with a shared vision. The ideal candidate will have:

  • Proven experience leading and transforming brokerage or commissioning services within a local authority, NHS, or integrated system.

  • Deep understanding of adult social care, hospital discharge processes, and continuing healthcare (CHC).

  • Demonstrable success in designing and delivering integrated services and sustainable joint funding arrangements.

  • Strong stakeholder engagement and communication skills, with the ability to navigate complex system dynamics.

  • A strategic mindset with a hands-on approach to implementation and change management.

Contract Details:
  • Initial Full-Time (5 days/week): First 2 months

  • Reduced Hours (2 days/week): Third month

  • Potential Extension: Up to 6 months total

  • Rate: £450 per day (via umbrella)
