Continuing Health Care Nurse Assessor – NHS Birmingham
Location: Birmingham
Hours: Full-time, Monday to Friday, 9am–5pm
Day Rate: £320 Ltd / £251.45 PAYE (excluding holiday pay)
IR35 Status: Inside IR35

Service Care Solutions are currently recruiting for a Continuing Health Care Nurse Assessor to join an NHS team in Birmingham.

Key Responsibilities:

  • Facilitate multi-disciplinary assessments and reviews in line with standard operating procedures.
  • Conduct continuing care assessments for children and young people across primary, secondary, social care, and educational settings.
  • Maintain effective relationships with internal and external stakeholders including social care, education services, and families.
  • Participate in transition meetings (16–18 and 18–25 years) to review care plans and support smooth transitions between services.
  • Contribute to joint commissioning for SEND, focusing on outcomes that matter most to children, young people, and their families.
  • Collaborate with professionals to assess complex needs and develop holistic care plans.
  • Support young people transitioning to adulthood by producing multi-agency plans for continued care or transition to adult services.
Requirements:

  • Registered Nurse with relevant experience in Continuing Health Care.
  • Strong understanding of multi-agency working and SEND frameworks.
  • Experience in conducting holistic assessments and care planning.
  • Knowledge of relevant legislation including the Children Act and Mental Health Act.
  • Ability to work independently and collaboratively within a multi-disciplinary team.
