Community Care Officer required in Doncaster for initial period of 3 months, with the possibility of extension after that. Varied and exciting position within the Integrated Discharge Team. Apply NOW to be considered!

An exciting opportunity has arisen for a Community Care officer to join the Integrated Discharge team. The successful candidate will provide a high quality, integrated, effective social work service to adults and carers in accordance with the Hospital Discharge and Community Support Guidance 2022, Care Act 2014 statutory requirements, Mental Capacity Act 2005, local policy, procedures and guidance.

The team works both within the acute setting at Doncaster Royal Infirmary in the Transfer of Care Hub and across the various discharge pathways to assess ongoing long term needs. The ethos of Home First should be considered as a starting point and aim for all of those people you will work with.

The Home First Directorate provides a 7 day service between the hours of 8am and 6pm and some weekend working is required of the successful applicant on a rotational basis with days off in the week. This is currently 1 in 7 to 1 in 8 weekends and can be discussed further.

The roles will involve undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs, manage risk and contribute to service planning. You will provide written and verbal records and reports which are concise, factual and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings.

You will use evidence based practice and a strengths based conversation to identify need and take appropriate action which is outcome focused. Progress made towards meeting outcomes will reviewed in a timely manner following discharge from hospital with an expectation that the level of need will reduce following the reablement support received.
