As a Community Nurse, you will play a crucial role in delivering high-quality care to patients within their homes, promoting independence and improving overall health outcomes.


Key Responsibilities:

  • Conducting comprehensive assessments of patients' health needs and developing personalised care plans.
  • Delivering a range of nursing interventions, including wound care, medication management, and symptom management.
  • Providing emotional support and practical advice to patients and their families.
  • Collaborating closely with multidisciplinary teams to ensure coordinated care delivery.
  • Maintaining accurate and up-to-date patient records and documentation.
  • Contributing to the evaluation and improvement of service delivery through feedback and reflection.

Requirements:

  • Registered Nurse (RN) qualification with active registration with the NMC
  • Previous experience working as a Community Nurse
  • Must be able to drive and have access to a vehicle.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients from diverse backgrounds.
  • A compassionate and empathetic approach to patient care.
  • Ability to work autonomously and make sound clinical judgments.
  • Flexibility to work across bases if needed
