Lead Complex Case Manager

Service Care Solutions is currently recruiting for a Lead Complex Case Manager on a temporary basis within the NHS. This is an exciting opportunity for an experienced professional to undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs, including retrospective and appeal cases.

The post holder will act as a Case Manager for individual clients within Birmingham Solihull ICB who are receiving continuing health care funding to assess the needs of the client and their family to facilitate, plan, and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environment.

The pay rates for this position are £200-£220 a day, and the role offers hybrid working/home/patient-facing options. In addition to the competitive pay rates, we offer a bonus scheme, which includes a £150 sign-up bonus and a £250 referral bonus.
