Service Care Solutions are currently recruiting for a Leaving Care Personal Advisor on behalf of Cardiff Council to provide young people leaving care with advice and guidance. You will assist young people in their participation in pathway planning to ensure that their pathway plans properly reflect their individual strengths, aspirations and needs. You will also need to make appropriate assessments of individual's needs in collaboration with the Children Looked After Social Workers and other agencies as required.
The successful candidate will ideally have experience of assessments, person centered planning and care management skills working actively with all partners to identify and meet individual's needs.You will also need to have team working skills, the ability to produce innovative solutions and be forward thinking to help develop the service. The ability to liaise with other professionals to be able to provide excellent outcomes for young people leaving care is also very important.
The role is a temporary, full time, position for an initial period of 3 months with a view to this being extended on a rolling basis. The role will be largely based at home due to the current Corona Virus restrictions but there will be some visits to the young people needed at times so as a result the successful candidate is required to be a car driver.
The ideal candidate will have:
* Experience of working with young people and their families
* Full UK driving license
* Enhanced DBS check
* Be able to work upon own initiative and within a team
