Job Role: To support vulnerable young people transitioning from care by managing complex cases involving assessment, planning, intervention, and coordination of services. Your work will align with the Children Act, national guidance, and local policies to help these young people achieve positive, independent futures.
Requirements for the Post
- GCSE Grade C or above (or equivalent) in English and Maths. Relevant professional qualification.
- Full UK driving licence (or reasonable adjustments for disabilities).
- Strong understanding of child development and challenges faced by care leavers. Familiarity with Children Act 1989/2004, Children (Leaving Care) Act 2000, and Children and Social Work Act 2017. Knowledge of safeguarding procedures.
- Experience (paid or voluntary) working with diverse young people. Ability to build supportive relationships and encourage independence. Experience in developing intervention strategies and managing challenging behaviours. Familiarity with case documentation and administrative processes.
- Confident using IT systems and digital recordkeeping.
- Committed to delivering excellent support and customer care. Strong interpersonal skills for building trust and collaboration. Sound judgment and problem-solving capabilities. Ability to work independently and in a team. Organised and able to prioritise under pressure. Dedicated to promoting equality, fairness, and safety.
Main Tasks:
- Manage a caseload of care leavers, carrying out assessments, planning, and reviews in line with legal and service requirements.
- Work collaboratively with multi-agency partners, attending reviews, planning meetings, and safeguarding forums.
- Maintain accurate case records and produce high-quality reports.
- Deliver flexible and practical support to care leavers, including life skills training, accommodation support, and transport assistance.
- Apply safeguarding principles and promote equality, diversity, and inclusion in all service areas.
- Participate in professional supervision, team meetings, and ongoing training to ensure best practice and up-to-date knowledge.
