As a Leaving Care Specialist (Personal Advisor), you will provide support services for children and young people aged 16–25 who are in care or have recently left care. You will help them navigate key areas of their lives such as education, training, employment, housing, and well-being. Your work will ensure that young people are safeguarded, empowered to make decisions, and supported in their transition to adulthood.

Key Duties May Include

  • Facilitating assessments and ongoing reviews of young people’s needs.
  • Providing practical, personal, and emotional support including domestic tasks and transportation.
  • Supporting young people with health, housing, benefits, education, and employment.
  • Developing Pathway Plans and Personal Action Plans tailored to individual needs.
  • Running group sessions and "drop-ins" as part of service delivery.
  • Working collaboratively with professionals, agencies, families, and communities.
  • Advocating on behalf of young people and promoting self-advocacy.
  • Supporting young people through transitions and crisis situations.
  • Maintaining accurate records and writing reports in line with policy.
Key Responsibilities
  • Promote and support the health, development, and well-being of care leavers.
  • Identify and help overcome barriers to learning, independence, and integration.
  • Encourage service user engagement in both personal planning and service development.
  • Support the development of young people’s independence and life skills.
  • Maintain interagency networks and knowledge of local resources.
  • Attend planning meetings, reviews, and legal proceedings as required.
  • Keep up to date with legislation, practice developments, and service procedures.
What They're Looking For
  • A relevant professional qualification (e.g., NVQ Level 3 or Level 4 equivalent in social care, youth work, teaching, or health).
  • Minimum of two years' continuous experience working with children and young people.
  • Experience of working with young people in supportive and empowering relationships.
  • Experience working in partnership with other agencies and professionals.
  • Understanding of housing, employment, and education challenges faced by young people.
  • Proven ability to communicate effectively and build trust with young people.
  • A full driving licence and access to a vehicle (or a driver for work purposes).
Skills & Competencies
  • Fluency in English with the ability to communicate complex information clearly.
  • Knowledge of safeguarding, child development, and transitions.
  • Effective use of IT systems including electronic case recording.
  • Ability to work both independently and as part of a team.
  • Flexible approach to working hours, including occasional evenings and weekends.
  • Competent in report writing and professional communication.
  • Understanding of relevant policies, procedures, and legal frameworks.
  • Commitment to personal and professional development.
Other Requirements
  • Enhanced DBS clearance is required.
  • Must be able to work in family homes and community settings, including unsocial hours.
  • Commitment to equality, dignity, and respect in service delivery.
  • Willingness to travel across the district as needed.
