Job Role
To act as a key point of contact for young people aged 16 to 25 leaving care, providing support, coordination, and intervention to promote successful transitions into adulthood. The role involves working directly with young people and their networks, delivering targeted support to address risks such as accommodation instability, unemployment, or mental health challenges. This includes contributing to planning, assessments, and delivering services in line with statutory responsibilities and local care strategies.
Requirements for the Post
- NVQ Level 3 or equivalent qualification.
- Full UK driving licence and access to a vehicle (reasonable adjustments considered).
- Strong understanding of the needs of care leavers aged 16 to 25 and of relevant legislation (e.g., Children Act 1989/2004, Children (Leaving Care) Act 2000, Children and Social Work Act 2017).
- Proven ability to build effective relationships with young people, their families, and professional networks.
- Experience in multi-agency working, case coordination, and producing pathway plans and reports.
- Confident using electronic case management systems and maintaining accurate documentation.
- Skilled in developing and delivering interventions around life skills, safeguarding, employment, education, and risk management.
- Committed to promoting equality, diversity, and inclusion, and delivering excellent customer service.
- Willingness to work flexibly, including evenings, weekends, and public holidays as required.
Main Tasks:
- Manage a caseload of care leavers, undertaking assessments and maintaining statutory Pathway Plans.
- Engage young people in decisions that affect their lives, empowering them to work toward independence.
- Provide practical support including household tasks, accommodation assistance, and access to education or employment.
- Deliver targeted interventions to reduce risks such as anti-social behaviour or poor mental health.
- Work collaboratively with partner agencies to deliver coordinated care and support.
- Maintain up-to-date case records and prepare reports that align with service requirements and timescales.
- Act as Lead Professional where appropriate, attending case conferences and planning meetings.
- Participate in a duty rota to provide crisis support, advice, and information to young people aged 21+.
- Attend regular supervision, training, and development sessions to maintain best practice.
