Leaving Care Worker
Location: Lincolnshire (with travel within/outside the county)
Client: Lincolnshire County Council
Salary: £17.30 per hour (Umbrella)
Contract Type: Temp (3 months)
Hours: Full-time, including evenings/weekends and on-call rota
Role Overview
As a Leaving Care Worker, you will act as a pivotal point of contact and coordination for care-experienced young people aged 16 to 25. Your role is central in ensuring successful transitions from care to independence, by providing consistent, flexible support and building trusting relationships. You will lead multi-agency planning, deliver targeted interventions, and advocate for young people to help them secure accommodation, access education, employment, and improve their health and wellbeing.
Key Duties May Include
  • Acting as the Lead Professional for care leavers, where appropriate
  • Direct work with young people, including practical and emotional support
  • Managing a caseload of young people and maintaining accurate records
  • Supporting young people in crisis through a duty service rota
  • Delivering interventions to reduce risks (e.g. offending, homelessness, mental health concerns)
  • Supporting access to education, training, and employment opportunities
  • Co-producing plans and strategies with young people to meet their goals
  • Involving families, peers, and wider networks in planning and support
Key Responsibilities
  • Build strong, trusting relationships with young people, even when they are reluctant to engage
  • Provide both support and challenge to promote lasting, positive change
  • Undertake assessments and coordinate care plans (Pathway Plans)
  • Liaise with a wide range of statutory and voluntary agencies
  • Participate in case conferences and meetings as directed
  • Prepare high-quality reports, contribute to planning reviews and ensure timely delivery of services
  • Ensure case management systems are maintained and up to date
  • Attend regular supervision, team meetings, and training
  • Work flexibly, including evenings, weekends, public holidays, and on-call
What They're Looking For
Essential:
  • NVQ Level 3 or equivalent qualification
  • Experience of supporting young people aged 16 to 25
  • Strong understanding of the needs of care leavers
  • Ability to build positive relationships with young people and professionals
  • Ability to produce accurate case records and reports
  • Confidence in acting as a Lead Professional in multi-agency settings
  • Awareness of safeguarding and child protection policies
  • Knowledge of relevant legislation and policy frameworks
  • Strong time management skills and flexibility in working patterns
  • Driving licence and access to a vehicle
Desirable:
  • Knowledge of child development
  • Understanding of health and safety policies
  • Experience working across children and adult services
  • Understanding of diversity, equality, and inclusion in service delivery
Skills & Competencies

  • Ability to coordinate services from multiple agencies
  • Strong interpersonal and communication skills

Other Requirements
  • Willingness to work evenings/weekends and participate in an on-call duty rota
