🔑 Position Overview:
We are seeking an experienced Occupational Therapist with a specialization in major adaptation to join our multidisciplinary team. The successful candidate will play a pivotal role in assessing service users, facilitating independence, managing complex health needs, and promoting holistic wellbeing.
📋 Key Responsibilities:
- Conduct initial assessments of service users with complex care and therapy needs across the First Response, Reablement, and Assessment Service teams.
- Support service users in setting client-centered goals to promote independence and wellbeing at home.
- Assess and provide equipment to improve independence in daily activities and reduce the risk of injury, including manual handling assessments.
- Independently manage a caseload, prioritizing effectively and managing time efficiently.
- Identify service users appropriate for Continuing Health Care to manage complex health needs.
- Identify and address Safeguarding concerns, adhering to Inter-Agency Policy and Procedures for Safeguarding Adults.
- Assess capacity and decision-making abilities as per the Care and Mental Capacity Act.
- Ensure cost-effective service provision and recommend specialist equipment and adaptations in line with eligibility criteria and departmental guidelines.
- Complete carers assessments to support informal carers and manage the risk of carer breakdown.
- Maintain accurate and up-to-date case records using council's IT systems and ensure compliance with statutory and local protocols.
- Uphold confidentiality regarding service users and information obtained during employment, in accordance with data protection regulations.
- Develop and maintain effective working relationships across Adult Social Care and other specialist agencies.
- Promote Occupational Therapy services to external organizations and provide guidance on service provision.
- Provide supervision and support for OT Assistants and junior team members, including training and professional development.
- Recommend changes in the Occupational Therapy role and service delivery as necessary and act as a skills resource for professionals across Adult Social Care and partner services.
🎓 Qualifications and Experience:
- Bachelor's or Master's degree in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Significant experience in occupational therapy with a focus on major adaptation.
- Knowledge of relevant legislation and best practices in adult social care.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
