Opportunity for a Qualified Physiotherapist to work within a Primary Care Community Team, Based in Nottingham

The role is based within the Primary Care Team with the role predominantly working on the project that will target 100 stable Heart Failure citizens, providing 'Breathlessness Rehabilitation' in their own home. The cases have been identified by PICS's existing specialist HF nurses through case load finding. This post will home visit the cases over the 16 week period. The citizens will receive a specialist 60-minute pre-assessment by this post. This will be a comprehensive holistic assessment with appropriate onward referral as required. Followed by six 60-minute sessions with the physiotherapist, once a week. These six physiotherapy sessions will involve appropriate rehabilitation advice and will seek to improve symptoms by restoring functional capabilities and enhance quality of life. The applicant will be responsible to updating the patient record of each case. Each citizen will receive a post assessment 60-minute session, where advice and guidance will be provided with clear personalized signposting to third sector organisations and self-help advice. The identified cases are in a localized area, enabling multiple home visits per day by the physiotherapist.

The role will be on a 16 Week Contract working 39 Hours Per Week.

The hourly rate for this role is negotiable based upon experience
