About the Role
The Specialist Support Worker will deliver inclusive, person-centred opportunities for adults with learning disabilities and complex or challenging needs. This includes supporting individuals in specialised community buildings and other settings, enabling participation in meaningful activities that promote independence, well-being, inclusion, and choice.
Post holders are expected to be flexible and may be asked to undertake duties that align with the general character and level of responsibility of the role.
Key Responsibilities
- Develop and review Person-Centred Care and Support Plans.
- Lead and implement individual and group activities in partnership with adults, their support networks, and key partners.
- Maintain accurate records and personal information in line with data protection standards.
- Provide personal care using specialist equipment and act as a designated link worker.
- Complete risk assessments, including moving with dignity assessments.
- Assist with medication administration and documentation.
- Support skill development including money management and reablement.
- Carry out transport duties and supervise individuals/groups.
- Ensure safeguarding policies are followed.
- Open and secure venues, adhering to health and safety procedures.
Experience & Knowledge
- Experience working with adults with complex and/or challenging needs.
- Understanding of person-centred care, community inclusion, and individual choice.
- Familiarity with national policies and local procedures (e.g. Safeguarding Adults, Mental Capacity Act).
- Knowledge of First Aid, Health and Safety, and Moving with Dignity.
- Experience in leading and delivering individual and group activities.
- Ability to compile and manage information using IT systems (e.g. Word, databases, email).
- Strong verbal and written communication skills.
- Effective engagement techniques with adults with complex needs.
- Problem-solving skills using various approaches.
- Ability to prioritise tasks and manage changing support needs.
- Creativity in developing support plans and contingency strategies.
- Ability to work independently and make decisions within delegated authority.
- Level 3 qualification in Health and Social Care or equivalent experience (or willingness to work towards).
- Ability to travel and work across North Lincolnshire.
- Physical capability to manage incidents, perform personal care, and transport duties.
- Comfortable working in varied environments including homes, community venues, and outdoors.
- Subject to DBS check under the Rehabilitation of Offenders (Exemption) Act 1974.
- Must be fluent in English (Immigration Act 2016 requirement).
