JOB TITLE: Support Worker
LOCATION: Guildford, Woking, and Waverley
CONTRACT TYPE: Temp
WEEKLY HOURS: 35 Hours
WORK PATTERN: Mon - Fri (9am-5pm)
JOB SUMMARY:
To deliver high quality person-centred support to customers living in supported accommodation service in accordance with the person-centred principles as a support officer in Newbury Area, enabling clients to sustain their tenancies by promoting independence, freedom, and choice.
RESPONSIBILITIES:

  • Assess referrals and engage with prospective clients using a person-centred approach. Key to this role will be liaising with health professionals, other agencies, and colleagues as appropriate.
  • Ensure that prospective and current clients are fully aware of the support available, their financial obligations, rights and responsibilities under their tenancy/occupation arrangements and all related procedures.
  • Flexibly deliver the support identified in an Independent Living Plan through regular person-centred contact as appropriate, taking follow up action and coordinating wraparound support if required from other agencies (for example Adult Social Care, Criminal Justice, Health services) and internal departments.
  • To empower clients to ensure the safety, security, and cleanliness of the project, including adherence with internal policies and procedures and reporting repairs where necessary.


EXPERIENCE:

  • A working knowledge and understanding of how to support people in achieving and maintaining independence.
  • Experience of providing support to diverse client groups.
  • Experience of working with clients experiencing substance misuse and mental health.
  • Ability to network and build strong relationships with partner agencies, community, and key stakeholders.


Also, will be required to go through an enhanced DBS check.
Driving is Essential.
