Adult Practitioner – our client & our client (37 hours/week) | Contract (2 weeks)

ServiceCare is recruiting for an Adult Practitioner to join an Adult Social Care team in our client & our client on a full-time contract for 2 weeks. If you’re passionate about supporting adults to live independent, fulfilling lives, the Adult Practitioner role offers the chance to make a real difference using strengths-based, person-centred practice.

Adult Practitioner – Candidate Responsibilities

  • Complete Care Act assessments, reviews, and reassessments as an Adult Practitioner
  • Undertake Carers Assessments to support individuals and their families
  • Carry out Mental Capacity Assessments and support Best Interests decision-making
  • Develop person-centred support plans that promote independence, wellbeing, and positive outcomes
  • Support the use of assistive technology, digital solutions, personal budgets, and direct payments
  • Work collaboratively with health professionals, community organisations, voluntary services, and partner agencies
  • Identify, manage, and mitigate risk while promoting positive risk-taking and informed choice
  • Maintain accurate case records and produce high-quality assessments and reports
  • Support safeguarding processes and promote the welfare of vulnerable adults
  • Contribute to quality assurance, service development, and continuous improvement as an Adult Practitioner

Adult Practitioner – What We’re Looking For

  • Experience working with adults with care and support needs
  • Strong understanding of the Care Act, Mental Capacity Act, safeguarding principles, and risk management
  • Knowledge of personal budgets and direct payments
  • Excellent communication and relationship-building skills
  • Ability to manage a varied caseload and work independently and within a multidisciplinary team
  • Level 4 qualification (or equivalent) and evidence of ongoing professional development (desirable)
