Adult Practitioner – Telford & Wrekin (37 hours/week) | Contract (2 weeks)
ServiceCare is recruiting for an Adult Practitioner to join an Adult Social Care team in Telford & Wrekin on a full-time contract for 2 weeks. Adult Practitioner – Candidate Responsibilities
- Complete Care Act assessments, reviews, and reassessments as an Adult Practitioner
- Undertake Carers Assessments to support individuals and their families
- Carry out Mental Capacity Assessments and support Best Interests decision-making
- Develop person-centred support plans that promote independence, wellbeing, and positive outcomes
- Support the use of assistive technology, digital solutions, personal budgets, and direct payments
- Work collaboratively with health professionals, community organisations, voluntary services, and partner agencies
- Identify, manage, and mitigate risk while promoting positive risk-taking and informed choice
- Maintain accurate case records and produce high-quality assessments and reports
- Support safeguarding processes and promote the welfare of vulnerable adults
- Contribute to quality assurance, service development, and continuous improvement as an Adult Practitioner

Adult Practitioner – What We’re Looking For
- Experience working with adults with care and support needs
- Strong understanding of the Care Act, Mental Capacity Act, safeguarding principles, and risk management
- Knowledge of personal budgets and direct payments
- Excellent communication and relationship-building skills
- Ability to manage a varied caseload and work independently and within a multidisciplinary team
- Level 4 qualification (or equivalent) and evidence of ongoing professional development (desirable)
