About the Role As an Adult Practitioner, you will work directly with adults who have care and support needs, helping them achieve positive outcomes through strengths-based assessments, support planning, and community-focused interventions.

Key responsibilities include:
- Completing Care Act assessments, reviews, and reassessments.
- Undertaking Carers Assessments.
- Completing Mental Capacity Assessments and supporting Best Interest decision-making.
- Developing person-centred support plans that promote independence and wellbeing.
- Supporting the use of assistive technology, digital solutions, personal budgets, and direct payments.
- Working collaboratively with health professionals, community organisations, voluntary services, and partner agencies.
- Identifying and managing risks while promoting positive risk-taking and individual choice.
- Maintaining accurate case records and producing high-quality assessments and reports.
- Supporting safeguarding processes and promoting the welfare of vulnerable adults.
- Contributing to service development, quality assurance, and continuous improvement initiatives.
- Experience working with adults with care and support needs.
- Experience undertaking assessments and developing support plans.
- A strong understanding of the Care Act, Mental Capacity Act, safeguarding principles, and risk management.
- Knowledge of personal budgets, direct payments, and strengths-based practice.
- Excellent communication and relationship-building skills.
- The ability to manage a varied caseload and work both independently and as part of a multidisciplinary team.
