Bolton Council’s Intermediate Care Team is seeking a Qualified Adults Social Worker to join their dedicated service. This is a fantastic opportunity to work in a fast-paced, supportive environment focused on strength-based practice and promoting independence.
The Role

• Undertake assessments and reviews under the Care Act 2014
• Complete support planning and connect individuals to universal and voluntary services
• Support complex assessments, including Mental Capacity and Best Interest assessments
• Work collaboratively within a multi-disciplinary team
• Build and maintain strong relationships with service users and carers

Requirements

• Diploma/Degree in Social Work
• Minimum 3 years’ reference history (gaps in employment checked)
• Relevant qualifications in line with JDPS
• Experience working with adults in a statutory setting
• Strong assessment, communication and partnership-working skills
