Advanced Nurse Practitioner (ANP) Service: Primary Care–Led Rehabilitation Unit
Location: Lings Bar, Gamston
Shift: Tuesdays, 10:00am – 4:30pm
Pay: £45 per hour (LTD)

About the Service This role sits within a Primary Care–led Rehabilitation Unit, delivering care to adults who require short-term inpatient rehabilitation. The service typically manages patients recovering from:

• Acute illness
• Exacerbations of long-term conditions
• Reduced mobility or functional decline

The unit provides essential step?down care for individuals discharged from acute hospitals but not yet ready to return home. The focus is on:

• Medical optimisation
• Functional recovery
• Effective discharge planning
• Reducing the risk of hospital re-admission

Although the setting is technically secondary care, the model is GP/ANP-led, not consultant-led, and care is delivered by a full multidisciplinary team, including:

• GPs / ANPs
• Nursing staff
• Physiotherapists & Occupational Therapists
• Pharmacy teams
• Discharge coordinators

Key Responsibilities As the ANP, you will play a crucial clinical role in managing patients within the unit. Duties include:

• Assessment and management of acute illness in inpatient adults
• Review and optimisation of long-term conditions
• Medication management, including drug charts and TTOs
• Clinical decision-making around escalation, ongoing care, or planning discharge
• Liaising with community teams, acute hospitals, and wider MDT members
• Supporting safe and timely discharge planning

What We’re Looking For

• NMC registered ANP
• Previous experience in Primary Care, community services, or inpatient rehab
• Confident working autonomously within a multidisciplinary environment
• Competent in SystmOne (essential)
• Strong clinical reasoning and communication skills
