Job Summary We are recruiting an experienced Advanced Nurse Practitioner (ANP) to support a Home Visiting Service across Mid Nottinghamshire. This is an excellent opportunity for a highly autonomous clinician who is confident managing patients within the community. The role is suited to an independent prescriber with strong assessment, diagnostic, and clinical decision?making skills.
You will be travelling to patients’ homes across Newark, Sherwood, Mansfield, and Ashfield to deliver safe, effective, high?quality care.

Key Responsibilities
- Conduct home visits to assess, diagnose, treat, and manage patients
- Prescribe independently in accordance with national and local guidelines
- Manage both acute presentations and long-term conditions autonomously
- Maintain accurate and timely clinical records using SystmOne
- Escalate or refer patients to appropriate services when clinically required
- Work collaboratively with local Primary Care teams, community providers, and other health professionals
- Qualified Advanced Nurse Practitioner (MSc L7 V300 Independent Prescribing)
- Proven ability to work autonomously in a community or home?visiting setting
- Confident in managing undifferentiated presentations
- Experience using SystmOne (S1)
- Full UK driving licence and access to own car
