About the Role:

This role requires a practitioner with strong communication skills and the ability to operate independently within clinical guidelines. Candidate Responsibilities:
- Conduct comprehensive assessments for adult patients, identifying their needs and developing tailored care plans.
- Provide expert advice and support to both patients and multidisciplinary teams.
- Ensure clinical documentation is complete, accurate, and compliant with organisational standards.
- Lead interventions and follow-up care, promoting best practices in adult health services.
- Maintain up-to-date knowledge of relevant legislation, policies, and clinical procedures.
- Participate in team meetings, training sessions, and quality improvement initiatives.
Candidate Requirements:
- Proven experience as an Advanced Practitioner within adult healthcare.
- Relevant registration and qualifications (e.g., NMC/HCPC registration).
- Strong assessment, decision-making, and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Flexibility to commit to a 2-month contract starting immediately.
