Are you passionate about supporting young people as they transition into independent adulthood? Trafford Council are looking for a dedicated and compassionate Aftercare Worker to join their Through Care service supporting care experienced young people.
Trafford’s innovative “through care” approach ensures children and young people receive consistent, relationship-based support from permanence through to independence. This is a fantastic opportunity to make a genuine impact on the lives of young people leaving care.
The Role As an Aftercare Worker / Personal Advisor, you will play a vital role in helping care leavers build independence, confidence, and stability within the community.

Key Responsibilities
- Carrying out the duties of a Personal Advisor under the Children Leaving Care Act 2000
- Supporting care experienced young people living independently in the community
- Building positive and trusting relationships with young people
- Working in partnership with external agencies and professionals to ensure the best outcomes possible
- Providing practical and emotional support tailored to individual needs
- Has experience working with children, young people, or vulnerable adults
- Understands the challenges faced by care leavers
- Has excellent communication and relationship-building skills
- Is passionate about improving outcomes for young people
- Can work effectively both independently and as part of a wider team
