Band 6 Community Psychiatric Nurse (CPN) – Dementia / Later Life Team Location: Gloucestershire

Service Care Solutions are recruiting on behalf of the NHS for a Band 6 Community Psychiatric Nurse (CPN) to join a Dementia / Later Life Team in Gloucestershire.

Contract Details

• Working hours: Monday to Friday, 9:00am – 5:00pm
• Pay rate: £26 per hour (Umbrella)
• Contract type: Temporary, 3 months initially, with the possibility of extension

Key Responsibilities

• Engage in complex mental health assessments and plan appropriate care in partnership with service users
• Undertake specialist assessments of complex nursing needs
• Utilise the Care Programme Approach (CPA) in line with Trust policy
• Provide specialist care, education, and advice to service users and carers experiencing common, severe, and enduring mental health conditions
• Act as Care Co?ordinator when required, including working with adults at risk and following supervision procedures
• Develop, implement, and evaluate care plans in collaboration with service users, carers, and other professionals
• Ensure all parties involved receive copies of care plans and that accurate records are maintained on Trust IT systems

Essential Requirements

• Registered Mental Health Nurse (RMN)
• Experience within Dementia, Older Adults, or Later Life services
• Strong knowledge of the Mental Health Act and legal aspects of mental health care
• Demonstrable skills in risk assessment and risk management
• Knowledge of different approaches to mental health assessment
