Candidate Responsibilities for the Band 6 CPN / Care Co-Ordinator Role

As a Band 6 CPN / Care Co-Ordinator, your main responsibilities will include:

• Conducting comprehensive mental health assessments with service users
• Developing and implementing care plans tailored to individual needs
• Coordinating with multi-disciplinary teams to ensure high standards of care
• Monitoring treatment progress and adjusting care strategies accordingly
• Providing advice and support to patients and their families
• Ensuring compliance with all relevant policies and procedures
• Documenting all care activities accurately and promptly
• Acting as a point of contact for service users, contributing to service development

Candidate Skills & Experience Required

The ideal candidate for the Band 6 CPN / Care Co-Ordinator role will have:

• Qualification as a Community Psychiatric Nurse (CPN)
• Proven experience working within mental health services
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and documentation skills
• Knowledge of relevant mental health legislation and policies
