Care Navigator Job Responsibilities

As a Care Navigator, your responsibilities will include:

• Providing administrative support to GP Practices and Primary Care Hubs
• Handling patient queries and offering information on services and procedures
• Working closely with healthcare professionals to ensure seamless patient care
• Maintaining accurate records and databases using EMIS
• Offering patient navigation duties and supporting patients throughout their journey

This Care Navigator role is ideal for individuals with previous experience as a Receptionist or Care Navigator within a GP or health centre. Your expertise will ensure patients receive the highest quality support and care.
