As a Care Navigator, you will be responsible for providing administrative support to GP Practices and Primary Care Hubs, ensuring that patients receive the best possible care. If you have previous experience as a Receptionist or Care Navigator within a GP or health centre, we would love to hear from you.

- Maintain accurate records and databases using EMIS
- Offer patient navigation duties and provide support to patients
- Competitive pay rate: £17 LTD p/h
