We’re Hiring: Case Assessor – Adult Social Care (Blackpool Area)

Are you passionate about helping adults live independently and safely in their communities? We’re looking for a dedicated Case Assessor to join our Adult Social Care team in the Blackpool area. This is a fantastic opportunity to make a real difference while working in a supportive and professional environment.

What you’ll be doing:

• Conducting assessments and reviews to support adults in maintaining independence.
  
  
• Developing, monitoring, and reviewing personalised care packages.
  
  
• Working closely with colleagues, providers, carers, and external agencies to deliver seamless care.
  
  
• Ensuring services comply with the Care Act (2014) and Mental Capacity Act (2005).
  
  
• Supporting and empowering individuals to make informed choices about their care.
  
  
• Advocating for vulnerable adults and ensuring safeguarding concerns are raised appropriately.
  
  

What we’re looking for:

• Experience working in social care and inter-agency settings.
  
  
• Knowledge of adult social care, the Care Act (2014), and relevant policies.
  
  
• Excellent communication, negotiation, and IT skills.
  
  
• Ability to build strong relationships with service users and colleagues.
  
  
• Strong record-keeping, problem-solving, and decision-making skills.
  
  
• GCSEs/NVQ Level 2 (or equivalent) including Maths and English.
