Job Title: Adult Social Worker N8 (Contract)

Position Overview:

The Adult Social Worker N8 will be responsible for supporting adults 4-10 weeks post-discharge, focusing on assessment and support planning. The role involves working closely with service users to ensure their needs are met through effective care planning, safeguarding, and assessment work, including MCA and CHC processes.

Key Responsibilities of the Adult Social Worker N8:

• Conduct post-discharge assessments and support planning for adults within the 4-10 week post-discharge period.
• Engage in safeguarding procedures and ensure protection of vulnerable adults.
• Facilitate Mental Capacity Act (MCA) assessments and best interests decisions.
• Involved in Continuing Healthcare (CHC) assessments and support planning.
• Maintain accurate documentation and record-keeping.
• Collaborate with multi-disciplinary teams and external agencies.
• Conduct community appointments and work flexibly between office and home-based settings.

Candidate Requirements:

• Recognized qualification in Social Work and valid HCPC registration.
• Experience working within adult social care, particularly post-discharge support.
• Strong knowledge of MCA, safeguarding, and CHC processes.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Flexible and adaptable approach to working environments.
