💷 PAYE: £279.41 per day
💷 Limited/Umbrella: £384.00 per day
📅 Contract Length:  up to 52 weeks 

Are you an experienced CHC Social Worker looking for a rewarding short-term contract opportunity in Somerset?
Service Care Solutions is currently recruiting for a skilled and motivated CHC Social Worker to join a dedicated team in Taunton. This is an excellent opportunity to utilise your expertise in Continuing Healthcare (CHC), supporting vulnerable individuals and ensuring they receive the care and services they need. Hybrid working 1 day a week in the office.


Key Responsibilities:

• Undertake comprehensive assessments to determine eligibility for Continuing Healthcare services.
• Develop and implement person-centred care plans.
• Work collaboratively with multidisciplinary teams, families, and partner agencies.
• Ensure compliance with relevant legislation, policies, and best practice guidance.
• Maintain accurate and timely case records.
• Respond effectively to urgent and complex situations to promote positive outcomes.

Requirements:

• Qualified Social Worker with current Social Work England registration.
• Previous experience within Continuing Healthcare (CHC).
• Strong understanding of social care legislation and assessment frameworks.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage competing priorities.
• Availability to commit to the full 2-week contract.
