You will:

  • Support safe and timely hospital discharges, working closely with healthcare and social care partners to ensure all necessary arrangements are in place.
  • Manage a diverse and sometimes complex caseload efficiently.
  • Maintain accurate, up-to-date records across multiple systems.
  • Handle sensitive information with professionalism and confidentiality.
  • Monitor provider performance to ensure standards of high-quality care are maintained.
  • Collaborate effectively with managers to improve processes and deliver better outcomes for service users.
