Continuing Health Care Nurse Assessor – Adults (NHS) Location: Northampton
Hours: Full time, Monday to Friday, 9:00am – 5:00pm
Day Rate: £320 Ltd / £251.45 PAYE (excluding holiday pay)
IR35 Status: Inside IR35

Service Care Solutions are currently recruiting for an experienced Continuing Health Care (CHC) Nurse Assessor to join an NHS service working with adult patients in Northampton. This is a full?time role offering a competitive rate and the opportunity to work within a supportive multi?disciplinary team.

Key Responsibilities

• Undertake Adult Continuing Health Care assessments in line with the National Framework
• Complete DSTs, checklists, reviews, and reassessments
• Facilitate and participate in multi disciplinary meetings
• Work collaboratively with health, social care, and commissioning teams
• Develop and review holistic, person?centred care plans
• Maintain accurate and up?to?date clinical documentation
• Support discharge planning and ongoing community care arrangements
• Liaise with service users, families, and professionals to ensure effective care delivery

Requirements

• Registered Nurse (Adult Nurse preferred)
• Previous experience working as a CHC Nurse Assessor with adults
• Strong understanding of the Adult CHC National Framework
• Experience in multi agency working
• Ability to work independently and manage workload effectively
• Excellent communication and organisational skills
