Job Summary Our established PCN is seeking a Qualified GP to join its collaborative, high-performing network of practices. This role offers an excellent opportunity for a developing GP to gain broad clinical exposure, strong support, and long-term stability.

Main Responsibilities
- Conduct face-to-face and telephone consultations for a wide range of clinical presentations
- Undertake home visits and nursing home reviews as required
- Manage repeat prescriptions, medication queries, and clinical documentation
- Screen patients for risk factors, early signs of illness, and chronic disease indicators
- Lead on clinical decision-making for undifferentiated and complex cases
- Request, interpret, and action diagnostic tests, ensuring appropriate follow-up
- Make timely referrals to secondary care or other healthcare providers
- Develop personalised care and treatment plans in line with NICE and local guidance
- Maintain accurate, high-standard clinical notes using EMIS or SystmOne
- Take part in clinical audits, learning events, and practice meetings
- Prescribe safely and appropriately, following formulary and BNF guidance
- Collaborate closely with the multidisciplinary team and wider Federation staff
- Contribute to PCN and Federation performance indicators and service priorities
- Confident in managing a broad primary care caseload
- Able to work flexibly across PCN sites using EMIS and/or SystmOne
- Strong communicator with a commitment to high-quality, patient-centred care
- Motivated to contribute to service improvement and local primary care development
