Health & Social Care Officer – Front Door Team (Adult Social Care) We are seeking a dedicated and confident Health & Social Care Officer to join a busy Front Door service supporting Adult Social Care referrals. This is a vital role acting as the first point of contact for vulnerable adults who may have complex, sensitive, or rapidly changing needs.
You will play a key part in ensuring that every referral is triaged efficiently, accurately, and with a person-centred approach, helping individuals access the right support at the right time.

You will play a key part in ensuring that every referral is triaged efficiently, accurately, and with a person-centred approach, helping individuals access the right support at the right time.
The Role Working within a fast-paced safeguarding and assessment environment, you will be responsible for:
- Receiving, processing, and triaging incoming adult social care referrals
- Handling safeguarding concerns, care support requests, and occupational therapy referrals
- Ensuring timely and accurate recording of information across systems
- Making informed initial decisions on prioritisation and onward pathways
- Liaising with internal teams and external partners to coordinate appropriate responses
- Supporting the delivery of an effective, responsive front door service for adults with a range of needs
- Managing situations that can be complex, unpredictable, and sometimes high-pressure
- Has a strong understanding of adult social care services and pathways
- Can confidently assess and prioritise information in a fast-moving environment
- Communicates clearly and compassionately, even in challenging situations
- Is confident working with vulnerable adults, including those with complex needs
- Works well both independently and as part of a multidisciplinary team
- Has experience in social care, safeguarding, referral management, or similar settings

Contract Details
- Hours: 37 per week
- Working pattern: Monday–Friday, 09:00–17:00
- Assignment length: Initial fixed-term agency cover
- Location: Nottingham area (office base required during induction period)
- Status: Inside IR35
