Inreach Worker – Camden, London Service Care Solutions are recruiting for an Inreach Worker to join Camden’s Routes off the Streets (RTS) team.

- Pay Rate: £23 Ltd per hour
- Hours: Full-time, 37.5 per week (Monday to Friday, 9:00 AM – 5:00 PM)
- Contract: Temporary (3 months) with potential for extension
- Start Date: Immediate
Key Responsibilities Case Management
- Carry a caseload of individuals placed in short-term temporary accommodation.
- Develop tailored support plans focused on recovery, stability, and long-term housing solutions.
- Maintain accurate and timely records of interactions and progress.
- Conduct comprehensive needs and risk assessments and make referrals to health and social care services.
- Build strong relationships with local accommodation providers.
- Act as a liaison between Camden RTS and accommodation services to ensure quality and consistency of support.
- Manage referrals into and out of short-term accommodation, ensuring smooth transitions.
- Work collaboratively with housing teams, support services, and external agencies to identify appropriate move-on options.
- Represent Camden RTS at partnership meetings and forums.
- Advocate for service users and contribute to borough-wide homelessness strategies.
- Attend MDTs and Team Around Me meetings to support and advocate for clients.
- Support RTS outreach and building-based services as needed.
- Respond to emerging needs and priorities, including coordinating emergency and extreme weather provision (SWEP).
- Experience working with people experiencing homelessness or complex needs.
- Strong understanding of housing pathways, temporary accommodation, and support services.
- Excellent communication and partnership-building skills.
- Ability to manage a caseload and work independently.
