Leaving Care Personal Advisor

Are you an experienced Family Practitioner seeking a rewarding role to make a real difference? We are currently recruiting for a Leaving Care Personal Advisor on a short-term contract basis. This is an excellent opportunity to contribute to supporting vulnerable young people transitioning out of care. Job Title: Leaving Care Personal Advisor
Employment Type: Initially 3 months
Location: Our client
Pay Rate: £28.79 umbrella an hour (£22.08 PAYE an hour)
 


Candidate Responsibilities:
As a Leaving Care Personal Advisor, your duties will include:

  • Providing tailored advice and support to young people aged 16-25 transitioning from care
  • Developing individual care plans in collaboration with the young people and relevant agencies
  • Promoting independence skills and life skills development
  • Monitoring progress, offering guidance on housing, education, employment, and health matters
  • Ensuring compliance with all statutory and organizational policies
  • Maintaining detailed records of interactions and outcomes
  • Acting as a dedicated advocate for the young people you support

Required Skills:

  • Proven experience as a Family Practitioner or in a similar role supporting vulnerable youth
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Knowledge of safeguarding policies and procedures
  • Compassionate approach with excellent organisational skills
