We are seeking a dedicated and compassionate Personal Advisor to join a committed Leaving Care Team, supporting young people as they transition from care into adulthood.

The Role As a Personal Advisor, you will build meaningful and trusting relationships with young people preparing to leave care, ensuring their voices are heard and their individual needs are at the centre of all planning and decision-making.
You will work closely with young people, partner agencies, and professionals to help care leavers develop the confidence, skills, and resilience needed to live independently and thrive in adulthood.

Key Responsibilities
- Build and maintain positive relationships with young people preparing to leave care.
- Participate in assessments, pathway planning, reviews, and preparation for independent living.
- Provide advice, guidance, and practical support around housing, education, training, employment, health, finances, and life skills.
- Coordinate services and interventions to ensure young people access the support available to them.
- Work collaboratively with partner agencies and professionals to promote positive outcomes.
- Maintain accurate and up-to-date case records using electronic recording systems.
- Respond flexibly to crisis situations and provide support when required.
- Act as an advocate for young people, promoting their rights, wellbeing, and safeguarding.
- Contribute to service development and work in line with local and national leaving care guidance.
Essential Requirements
- Good general education to NVQ Level 3 or equivalent.
- A strong understanding of the challenges and issues facing young people leaving care.
- Excellent communication and interpersonal skills.
- The ability to build effective working relationships with young people, colleagues, and partner agencies.
- Strong organisational and time management skills.
- Good IT skills and experience maintaining accurate records.
- A commitment to equality, diversity, inclusion, and outstanding customer care.
- Flexibility to work occasional evenings and weekends when required.
- The ability and willingness to travel across the borough.
- NVQ Level 3 in Advice and Guidance.
- NVQ Level 3 in Health and Social Care.
- Knowledge of education, training, and employment pathways available to young people.
- Experience of assessment and support planning with young people.
