About the Role:

Your professional expertise will enable you to support service users in achieving meaningful goals while promoting their independence and wellbeing.

Candidate Responsibilities:

• Conduct comprehensive assessments of service users' physical and mental abilities
• Design and implement personalised reablement plans aligning with individual goals
• Guide and motivate individuals through recovery processes
• Offer expert advice on equipment and safe practice to both service users and colleagues
• Collaborate closely with a multidisciplinary team to ensure optimal support
• Promote a positive, respectful environment fostering trust and independence
• Stay updated with best practices and contribute to continuous service improvements
• Act as a supporting and trusted advisor to team members and service users alike

Candidate Requirements:

• Qualified Occupational Therapist with relevant registration
• Experience in reablement, community, or adult social care settings (desirable)
• Strong assessment and communication skills
• Compassionate, adaptable, and patient-focused approach
• Ability to work effectively within a team and independently
