As an Occupational Therapist Reablement Ca11 - 1331 Grade K, you will be responsible for managing pathway 1 hospital discharges, ensuring smooth transitions for patients. Working in a fast-paced yet friendly environment based at County Hall, Trowbridge, you will assess patients on their day of discharge, develop tailored support plans, and write comprehensive assessments to facilitate effective reablement.

Candidate Responsibilities for the Occupational Therapist Reablement Ca11 - 1331 Grade K

• Conduct assessments for patients immediately on discharge from hospital, focusing on their current needs and support requirements
• Develop and implement personalized support and rehabilitation plans to promote independence
• Collaborate with multidisciplinary teams to ensure continuity of care and smooth patient transitions
• Document all assessments and support plans accurately and promptly
• Provide advice and guidance to patients and their families, supporting ongoing recovery and well-being
• Work efficiently within a dynamic environment, managing a caseload with professionalism and empathy

Candidate Requirements for the Occupational Therapist Reablement Ca11 - 1331 Grade K

• Registered with the Health and Care Professions Council (HCPC)
• Proven experience in hospital discharge and reablement services
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Strong organisational skills and attention to detail
