Service Care Solutions are recruiting for an Outreach Worker to join a dedicated team in Northampton. This is a full-time role for approximately 3 months, with the possibility of extension.
Key Responsibilities:
- Engage with individuals experiencing street-based living, including rough sleepers and beggars, to support them into more stable lifestyles.
- Empower service users through increased choice and involvement, working collaboratively to achieve sustainable change and positive outcomes.
- Build on service users’ personal strengths and social networks, both in the UK and in their country of origin.
- Deliver flexible and effective interventions to engage hard-to-reach individuals.
- Conduct outreach shifts at various times and locations to rapidly assist rough sleepers.
- Provide assertive outreach to entrenched rough sleepers, including those initially unwilling to engage.
- Support timely hospital discharges by securing appropriate accommodation for homeless service users.
- Collaborate with drug and alcohol teams to ensure access to treatment for substance misusers.
- Offer advice using recognised models such as Motivational Interviewing to help service users explore and address substance misuse.
- Work with mental health agencies to ensure access to appropriate support.
- Facilitate ongoing healthcare access for service users.
- Experience of working as part of an outreach team
- Experience of working with rough sleepers
- Experience of working in a social care environment
- Experience of working within the substance misuse sector
- Knowledge of local services and geography
