Personal Adviser – Leaving Care Team Multiple Positions Available

We are looking for experienced and compassionate Personal Advisers to join our Leaving Care Team, supporting care-experienced young people aged 16–25 as they transition into adulthood and independent living.

- Accommodation and independent living
- Education, employment and training
- Emotional health and wellbeing
- Life skills and personal development
- Accessing services and community support
- Develop, review and update Pathway Plans
- Support young people to develop independence and resilience
- Work closely with social care, housing, education providers and partner agencies
- Advocate for young people and ensure their voices are heard
- Deliver flexible, person-centred support based on individual needs
- Have experience working with vulnerable young people or care-experienced young adults
- Are skilled at building positive, trusting relationships
- Can communicate effectively and work collaboratively with partner agencies
- Are compassionate, proactive and solution-focused
- Can manage a varied caseload independently while contributing positively to a team environment
