Personal Adviser – Leaving Care Team Multiple Positions Available

Are you passionate about supporting young people to build independent, successful futures?

We are looking for experienced and compassionate Personal Advisers to join our Leaving Care Team, supporting care-experienced young people aged 16–25 as they transition into adulthood and independent living.

This is a rewarding opportunity to make a genuine difference in the lives of young people by providing practical guidance, emotional support, and consistent encouragement during one of the most important stages of their journey.

The Role As a Personal Adviser, you will build trusted relationships with young people and provide tailored support to help them achieve positive outcomes across all areas of their lives, including:

• Accommodation and independent living
• Education, employment and training
• Emotional health and wellbeing
• Life skills and personal development
• Accessing services and community support

You will also:

• Develop, review and update Pathway Plans
• Support young people to develop independence and resilience
• Work closely with social care, housing, education providers and partner agencies
• Advocate for young people and ensure their voices are heard
• Deliver flexible, person-centred support based on individual needs

About You We are looking for motivated and resilient individuals who:

• Have experience working with vulnerable young people or care-experienced young adults
• Are skilled at building positive, trusting relationships
• Can communicate effectively and work collaboratively with partner agencies
• Are compassionate, proactive and solution-focused
• Can manage a varied caseload independently while contributing positively to a team environment
