Post 14 Case Officer (SEND & Inclusion) 

Are you an experienced Post 14 Case Officer with a passion for supporting young people with special educational needs and disabilities (SEND)? We are looking for a dedicated professional to join our SEND & Inclusion Service on a short-term contract. This is an excellent opportunity to make a real difference while working within a dynamic and supportive team.

Job Title: Post 14 Case Officer
Employment Type: Contract
Duration: Initially 3 months
Location: Hybrid (Chatham Office + our client Hubs)
Rate: £340 per day (umbrella) (£240 PAYE per day)

Candidate Responsibilities:
As a Post 14 Case Officer, your key responsibilities will include:

  • Managing the end-to-end process of Education, Health and Care Plans (EHCPs) and annual reviews for young people aged 14–25 with SEND.
  • Ensuring EHCPs are person-centred, compliant with the Children and Families Act 2014 and SEND Code of Practice (2015).
  • Working collaboratively with young people, families, schools, further education, health professionals, and social care colleagues.
  • Supporting the development of personalised, outcome-focused EHCPs that reflect individual needs and aspirations.
  • Maintaining accurate case records, monitoring progress, and identifying service improvement opportunities.
  • Providing guidance and support to families throughout the EHCP process.

Essential Skills & Requirements:

  • Proven experience working with post-14 SEND cases.
  • Demonstrable experience managing and maintaining EHCP plans effectively.
  • Previous experience within a local authority SEND or Education service.
  • Ability to work effectively in a hybrid environment, attending the Chatham office three days per week (please note temporary location at Pembroke Court, University of Kent Campus, Chatham).
