Practice Nurse - 12 month Fixed-Term Contract

Service Care Solutions are recruiting for an experienced and autonomous Practice Nurse to support patients with Long-Term conditions. This is an exciting opportunity to support an established service in the North West.

Location: Blackpool FY4

Pay: Dependent on Experience
System: EMIS

Hours: Full-time or Part-Time | Up to 37.5hrs per week


The Role
We are seeking a motivated Practice Nurse to join a community-focused service, delivering high-quality, patient-centered care across Blackpool. The successful candidate will support patients at risk of unplanned hospital admissions, particularly those with long-term conditions such as cardiovascular disease (CVD) and COPD.

 
Key Responsibilities

• Deliver proactive reviews for patients with long-term conditions (CVD/COPD)
• Undertake holistic assessments covering physical, mental, and social needs
• Provide health promotion, lifestyle advice, and preventative care
• Identify and respond to escalating health concerns
• Promote self-management and patient engagement
• Maintain accurate clinical records using EMIS
• Work autonomously while collaborating with the wider MDT

About You

• Minimum 2 years post-registration NMC Registration experience
• Experience in primary care or long-term condition management
• Strong clinical assessment and communication skills
• Ability to work independently and adapt in a community setting
• Non-Medical Prescriber (V300) - Desirable
