Practice Nurse – Primary Care Centre, Burgess Hill
Service Care Solutions are recruiting for an experienced Practice Nurse to support a Primary Care Centre in Burgess Hill on a part-time basis. This is an excellent opportunity for a skilled clinician with strong experience in Chronic Disease Management and Wound Care to join a supportive primary care team.
Contract: Part-time – (minimum 30 hours over 3 days)
Location: Burgess Hill
Hours: Flexible across 3 days
Pay: £32 ph ltd.

Job SummaryThe Practice Nurse will work as part of the primary care team to provide high-quality nursing care to the practice population. The role will focus on delivering chronic disease management clinics, wound care services, and supporting patients to improve and maintain their health and wellbeing.
Clinical Duties
- Deliver routine nursing care and treatment to patients within agreed clinical protocols.
- Undertake chronic disease management clinics, including diabetes, asthma, COPD, hypertension, and cardiovascular disease reviews.
- Perform health assessments and NHS Health Checks.
- Administer routine vaccinations in accordance with national guidelines.
- Carry out cervical cytology screening.
- Provide wound care, dressings, and management of minor injuries.
- Undertake venepuncture, ECGs, spirometry, blood pressure monitoring, and other clinical procedures as required.
- Promote health education and support patients in managing long-term conditions effectively.
- Registered Nurse with current NMC registration and experience working within a GP Surgery.
- Experience working within General Practice or Primary Care.
- Strong experience in Chronic Disease Management and Wound Care.
- Competent in immunisations, cytology, and wound dressing.
- Confident using EMIS
