Programme Planner (Reablement) – Contract

A leading care provider is seeking a Programme Planner to join a dynamic Reablement team on a contract basis. This role is essential to ensuring person-centred support is delivered on time, with accurate rotas and effective scheduling. Programme Planner Responsibilities

As a Programme Planner, you will:

• Organise and schedule daily visits for assessors, assistants and therapists in line with individual outcome-focused goals
• Maintain accurate, up-to-date rotas to ensure staff are in the right place at the right time
• Coordinate changes to planned care visits (e.g., cancellations, service adjustments, urgent needs)
• Monitor and support timely delivery of short-term Reablement packages (typically up to six weeks)
• Liaise with internal teams to ensure schedules reflect current priorities and service requirements
• Keep operational information organised, ensuring smooth day-to-day running of the service

Pay Rate

Pay rate: £14.00–£17.00 per hour (dependent on experience and suitability).
