Job Title: QSW - Adults - Community Care (Learning Disabilities)

Are you an experienced Qualified Social Worker (QSW) specialising in adult community care, particularly within learning disabilities? We are currently seeking a dedicated QSW - Adults - Community Care (Learning Disabilities) for a 6-week contract. This role offers an excellent opportunity to make a meaningful difference within the community, working with vulnerable adults to ensure they receive the highest standard of care.

About the Role: QSW - Adults - Community Care (Learning Disabilities)

As a QSW - Adults - Community Care (Learning Disabilities), you will be responsible for assessing, planning, and supporting adults with learning disabilities to live independent and fulfilling lives. The successful candidate will work closely with individuals, their families, and multidisciplinary teams to develop personalised care plans. This position requires you to attend the office three days a week, with the flexibility to work remotely on the remaining days.

Candidate Responsibilities for the QSW - Adults - Community Care (Learning Disabilities) role include:

  • Conduct thorough assessments of adults with learning disabilities and complex needs
  • Develop and implement tailored care plans to support independence and well-being
  • Work collaboratively with health and social care professionals, families, and carers
  • Ensure all interventions are in line with legal, regulatory, and organisational policies
  • Maintain accurate case records and document all interactions in compliance with GDPR and organisational standards
  • Attend three days per week in the office to facilitate team collaboration and case reviews
  • Stay informed of best practices and legislative requirements relevant to adult social care

Candidate Skills & Requirements for the QSW - Adults - Community Care (Learning Disabilities):

  • Qualification as a Qualified Social Worker (QSW) or equivalent
  • Experience working within community adult social care, specifically with learning disabilities
  • Knowledge of legislation surrounding adult social care, safeguarding, and mental capacity
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid UK driving license (preferred but not essential)
