Recovery Outreach Practitioner - Brent

We are seeking a dedicated Recovery Outreach Practitioner to join our team in Brent on a 3-month contract. Candidate Responsibilities:

As a Recovery Outreach Practitioner, your key responsibilities will include:

  • Delivering high-quality, person-centred, low-threshold street-based interventions to individuals who are rough sleeping or at risk of homelessness
  • Managing a small, complex caseload including assessments, evidence-based interventions, recovery planning, and aftercare support
  • Conducting street outreach to engage and re-engage homeless individuals misusing substances, building trusting relationships with street-active people
  • Providing psychosocial one-to-one interventions such as harm reduction advice, overdose prevention (including naloxone), blood-borne virus education, and Opiate Replacement Therapy
  • Facilitating access to mutual aid, education, employment, health, and wellbeing services, along with signposting to additional community resources
  • Working collaboratively with multidisciplinary teams, housing providers, and local authorities to support service users into treatment and reduce street drinking and anti-social behaviour
  • Maintaining accurate, timely case records using designated management systems in compliance with data protection policies
  • Actively participating in multi-disciplinary review meetings and safeguarding activities, ensuring adherence to organisational and statutory safeguarding policies

Candidate Requirements:

  • Experience working with substance misuse services or socially excluded/vulnerable groups
  • Ability to work calmly under pressure and meet deadlines
  • Excellent communication and interpersonal skills, with a team-oriented approach
  • Demonstrable ability to deliver high-quality service to people with complex needs
  • Strong IT skills, including Microsoft Office, digital platforms, and case management systems (e.g., Nebula, System One)
  • NVQ/QCF/RCF Level 3 in Health and Social Care or equivalent (or willingness to complete within 2 years)
  • Knowledge of relevant health and social care regulatory frameworks (CQC, NICE, etc.)
  • Valid driver’s license is desirable but not essential
