Short Breaks Personalisation Officer – Children’s Short Breaks Service (Contract – 2 Weeks)

We are seeking a dedicated Short Breaks Personalisation Officer to join Nottingham City Council’s Children’s Short Breaks Service within the Whole Life Disability Team. About the Role – Short Breaks Personalisation Officer

As a Short Breaks Personalisation Officer, you will work closely with children, young people, and their families to develop and coordinate tailored support that aligns with their unique needs and aspirations. Your day-to-day responsibilities will include facilitating access to Short Breaks Services, assisting with applications, and conducting reviews to ensure positive outcomes. This role calls for a compassionate individual committed to empowering families and advocating for inclusive, meaningful activities.

Candidate Responsibilities – Short Breaks Personalisation Officer

  • Support disabled children, young people, and families to access appropriate short break opportunities and community activities
  • Develop creative, person-centred support plans based on individual outcomes and preferences
  • Process applications and referrals for Pathway 1 and Pathway 2 Short Breaks provision
  • Provide clear information, guidance, and advice on available services and community resources
  • Empower families to explore and utilise support options, including direct payments where suitable
  • Conduct annual reviews to ensure services are effective and meet changing needs
  • Collaborate with education providers, health professionals, and partner agencies
  • Contribute to Education, Health and Care Plans (EHCPs) through reports and discussions
  • Identify safeguarding concerns and escalate risks appropriately
  • Assist in service development, data collection, and quality improvement initiatives
  • Represent the service at meetings, presentations, and multi-agency events

Candidate Requirements – Short Breaks Personalisation Officer

  • Experience working with children and young people with disabilities and their families
  • Knowledge of person-centred planning and outcome-focused support strategies
  • Understanding of safeguarding practices and children’s services
  • Ability to build positive relationships with families, professionals, and community providers
  • Strong communication, organisational, and problem-solving skills
  • Passionate about promoting independence, inclusion, and choice
  • Capable of managing a varied caseload and prioritising workload effectively
  • Ability to work independently and as part of a team
