? We’re Hiring: Social Care Assessors – Multiple Roles Available in Manchester! ?

Are you passionate about making a real difference in people’s lives? We’re looking for Social Care Assessors to join our team in Manchester, helping vulnerable adults and families access the right support to live healthier, more independent, and fulfilling lives.

As a Social Care Assessor, you’ll:
? Undertake holistic, strengths-based assessments in the community, hospitals, and intermediate care facilities.
? Arrange support such as reablement, care packages, respite, equipment, minor adaptations, and assistive technology.
? Develop and implement personalised support plans to meet social care needs.
? Identify safeguarding and mental capacity issues, referring appropriately.
? Collaborate with partner agencies to promote independence and positive outcomes.

What we’re looking for:

• Strong communication, analytical, and planning skills.
  
  
• Ability to work collaboratively across multiple teams.
  
  
• Commitment to continuous learning and self-development.
  
  
• NVQ Level 4 in Health and Social Care (or willing to achieve within 6 months).
  
  
• Enhanced DBS clearance (or willingness to obtain).
