As a Social Care Assessor, you’ll:
? Undertake holistic, strengths-based assessments in the community, hospitals, and intermediate care facilities.
? Arrange support such as reablement, care packages, respite, equipment, minor adaptations, and assistive technology.
? Develop and implement personalised support plans to meet social care needs.
? Identify safeguarding and mental capacity issues, referring appropriately.
? Collaborate with partner agencies to promote independence and positive outcomes.

- Strong communication, analytical, and planning skills.
- Ability to work collaboratively across multiple teams.
- Commitment to continuous learning and self-development.
- NVQ Level 4 in Health and Social Care (or willing to achieve within 6 months).
- Enhanced DBS clearance (or willingness to obtain).
- Be part of a team that puts people at the heart of what we do.
