Social Worker – Adults Mental Health (NHS)
Location: London
Contract: 3 months ongoing
Hours: Monday to Friday, 9:00am – 5:00pm
Working Pattern: Hybrid – 3 days office?based / 2 days working from home
Pay Rate: £32.69 per hour (Umbrella)

Service Care Solutions are recruiting on behalf of one of our NHS clients in London for an experienced Social Worker to join an Adults Mental Health Team.

Key Responsibilities:

• Managing a caseload of adults with mental health needs
• Completing assessments, reviews, and care planning in line with legislation
• Working collaboratively within a multidisciplinary team
• Supporting hospital discharge planning and community pathways
• Maintaining high?quality case recording in line with NHS standards

Requirements:

• Qualified Social Worker
• Relevant professional registration
• Experience within Adults Mental Health services
• Ability to work effectively within an NHS or multidisciplinary setting
